Woman resting a hand on her chest at home, a common sign of breast tenderness before period

What Causes Breast Tenderness Before Period?

Your work blouse suddenly feels tight, and even a hug from your kid makes you wince. If this happens a few days before your period every month, you are dealing with breast tenderness before period — one of the most common monthly complaints women have.

Breast tenderness before period means both breasts feel sore, swollen or heavy in the days leading up to your period, then settle once it starts. Doctors call it cyclical breast pain. Cleveland Clinic notes that around 7 in 10 women get breast pain at some point, and most causes are not serious.

What causes breast tenderness before period?

The short answer is hormones. Your cycle runs on two main ones, and both act on breast tissue:

  • Oestrogen rises in the first half of the cycle and makes the milk ducts grow.
  • Progesterone peaks after ovulation and makes the milk glands swell.
  • Fluid retention in the week before your period adds extra fullness and pressure.
  • Prolactin may also play a part, which is why some women feel it more than others.

When these signals stack up in the second half of the cycle, the tissue becomes fuller and more sensitive. Once your period begins, hormone levels drop, the swelling goes down and breast tenderness before period fades.

Timing: when breast tenderness before period starts and stops

Most women notice it about a week before their period, sometimes up to two weeks. It is part of the same pattern as other PMS signs. Cleveland Clinic's PMS guide lists breast soreness next to bloating, tiredness and acne, and says almost half of women report at least one PMS symptom.

A simple check: if the soreness fades within a day or two of bleeding and returns at the same point next month, that is the classic cyclical pattern.

What breast tenderness before period usually feels like

  • A dull ache or heaviness in both breasts
  • Soreness that spreads towards the armpit
  • Breasts feeling fuller, lumpier or bigger than usual for a few days
  • Discomfort when you go down the stairs, run for the LRT or lie on your front

Severe pain is uncommon. Most women describe breast tenderness before period as annoying rather than unbearable.

Things that can make it worse

1. A bra that no longer fits

Breasts change with weight, age and pregnancy. A bra bought three years ago may not give enough support now, so every movement tugs on already-sensitive tissue.

2. Lots of kopi, teh and soft drinks

Caffeine is a common suspect. Evidence is mixed, but many women find two cups of kopi a day feels very different from four.

3. Salty food in PMS week

Instant noodles, keropok and soy-sauce-heavy dishes encourage water retention. That extra fluid can add to the swollen, tight feeling.

4. Hormone medicines

Birth control pills, fertility treatment and hormone therapy can change how your breasts feel. Never stop or change these without speaking to your doctor first.

5. Stress and short sleep

Late nights and work deadlines tend to make every PMS symptom feel louder, breast tenderness before period included.

When breast pain is not the usual monthly kind

Cyclical soreness is common and, according to an NHS hospital breast clinic, breast pain on its own is not linked to a higher risk of serious disease. Still, see a doctor if you notice:

  • A new lump or a lumpy area that stays after your period
  • Pain in one spot or one side that does not follow your cycle
  • Nipple discharge, especially if it is bloody
  • Pain that lasts more than two weeks or keeps you awake
